When you were submitting a record, you may have accidentally uploaded the wrong file.
You will only be able to remove records that are APPROVED or PENDING.
Here are the steps to remove the records from your account:

Login to your account. Click on Click Here to access the To-Do List of your account.
On the To-Do List page, next to the requirement, under the Answer column, you will see Edit and View Document.
A notification box will appear with a warning that this action cannot be undone. Click Change Answer.
From the Saved Successfully page your will be able to click Close and then upload the correct document into your account.
Tip: This feature does not delete the record from your account. It removes it from the school’s view. The record will still be located in your medical record archive, which is located on your Front Desk.
Note: This feature is only available if uploading is permitted as an option to submit records.
